    If lead professional unable to attend CPA, all efforts will be made to find a representative from the team
Patient and Carer input and involvement throughout

  • Attempt to avoid rapid discharges if possible.  Discharge should be planned from day one of admission
  • Community team to be informed of day of discharge to facilitate 72 hour follow up. Where possible, community team to attend discharge meeting
